                <content:encoded><![CDATA[<p>Sleep is one of the richest topics in science today: why we need it, why it can be hard to get, and how that affects everything from our athletic performance to our income. <b>Daniel Kripke</b>, co-director of research at the <span class="yshortcuts" id="lw_1213128712_0">Scripps Clinic</span> Sleep Center in La Jolla, Calif., has looked at the most important question of all. In 2002, he compared death rates among more than 1 million American adults who, as part of a study on cancer prevention, reported their average nightly amount of sleep. To many his results were surprising, but they've since been corroborated by similar studies in <span style="border-bottom: 1px dashed rgb(0, 102, 204); cursor: pointer;" class="yshortcuts" id="lw_1213128712_1">Europe</span> and East Asia. Kripke explains.</p><p><b>Q:</b> How much sleep is ideal?</p> <p><b>A:</b> Studies show that people who sleep between 6.5 hours and 7.5 hours a night, as they report, live the longest. And people who sleep 8 hours or more, or less than 6.5 hours, they don't live quite as long. There is just as much risk associated with sleeping too long as with sleeping <span class="yshortcuts" id="lw_1213128712_2">too short</span>. The big surprise is that long sleep seems to start at 8 hours. Sleeping 8.5 hours might really be a little worse than sleeping five.</p> <p><span style="border-bottom: medium none; background: transparent none repeat scroll 0% 50%; cursor: pointer; -moz-background-clip: -moz-initial; -moz-background-origin: -moz-initial; -moz-background-inline-policy: -moz-initial;" class="yshortcuts" id="lw_1213128712_3">Morbidity</span>, [or sickness,] is also "U-shaped," in the sense that both very short sleep and very long sleep are associated with many illnesses - with depression, with obesity, and therefore with <span style="border-bottom: 1px dashed rgb(0, 102, 204); cursor: pointer;" class="yshortcuts" id="lw_1213128712_4">heart disease</span> and so forth. But the [ideal amount of sleep] for different health measures isn't all in the same place. Most of the "low points" are at seven or eight hours, but there are some at six and some even at nine. I think diabetes is lowest in seven-hour sleepers, [for example]. But these measures aren't as clear as the mortality data.</p> <p>I think we can speculate [about why people who sleep 6.5 to 7.5 hours live longer], but we have to admit that we don't really understand the reasons. We don't really know yet what is cause and what is effect. So we don't know if a short sleeper can live longer by extending their sleep, and we don't know if a long sleeper can live longer by setting the alarm clock a bit earlier. We're hoping to organize tests of those questions.</p> One of the reasons I like to publicize these facts is that I think we can prevent a lot of insomnia and distress just by telling people that short sleep is OK. We've all been told you ought to sleep eight hours, but there was never any evidence. A very common problem we see at sleep clinics is people who spend too long in bed. They think they should sleep eight hours or nine hours, so they spend eight or nine hours in bed, with the result that they have trouble falling asleep and they wake up a lot during the night. Oddly enough, a lot of the problem [of insomnia] is lying in bed awake worrying about it. There have been many controlled studies in the United States, Great Britain and other parts of <span style="border-bottom: medium none; background: transparent none repeat scroll 0% 50%; cursor: pointer; -moz-background-clip: -moz-initial; -moz-background-origin: -moz-initial; -moz-background-inline-policy: -moz-initial;" class="yshortcuts" id="lw_1213128712_5">Europe</span> that show that an insomnia treatment that involves getting out of bed when you're not sleepy, and restricting your time in bed, actually helps people to sleep more.                 <content:encoded><![CDATA[<p>A Manila hospital has closed its neonatal intensive care unit and an investigation ordered after 25 babies died of a blood infection, it was reported Saturday. </p><p>Local authorities ordered the Ospital ng Makati to close the unit after 45 babies contacted neonatal sepsis last month.   </p><p>The Philippine Daily Inquirer said the Department of Health had launched an investigation while the city's mayor, Jejomar Binay, has asked for a report on the deaths by Monday. </p><p>The unit was being sterilised on Saturday and number of women giving birth in the public hospital restricted.   </p><p>The Inquirer said 45 cases of neonatal sepsis, a blood infection contracted by infants less than 90 days old, were reported at the hospital in May out of 273 deliveries. </p><p>Eric Tayag of the department of health's National Epidemiology Centre told the Inquirer that infections could occur if the mother had poor immunity to diseases or did not undergo prenatal checkups. </p><p>"Neonatal sepsis could also be caused by unsanitary hospital conditions," he added.   </p><p>Tayag said 25 deaths in one month was "alarming."   </p><p>The hospital had 16 cases of sepsis in the four months to April but did not report any deaths.   </p><p>Two years ago another Manila hospital was forced to close its neonatal unit after seven babies died of sepsis.   </p>]]></content:encoded>

Rivara, M.D., M.P.H., et al</em><br /> Most patients have moderately severe pain resulting from their injuries one year after sustaining major trauma, according to a report in the March issue of Archives of Surgery, one of the JAMA/Archives journals. <p> &ldquo;Pain is a natural accompaniment of acute injury to tissues and is expected in the setting of acute trauma,&rdquo; according to background information in the article. Recent studies have shown that most patients with pelvic fractures and lower extremity injuries continue to experience chronic pain five to seven years after injury. Pain after injury can lead to disability, post-traumatic stress disorder and depression. </p><p> Frederick P. Rivara, M.D., M.P.H., of the University of Washington, Seattle, and colleagues analyzed information from 3,047 patients (age 18 to 84) who were admitted to the hospital and survived to one year after experiencing acute trauma. Pain 12 months after injury was measured on a 10-point scale. Personal, injury and treatment factors that may predict chronic pain in these patients were also noted. </p><p> &ldquo;At 12 months after injury, 62.7 percent of patients reported injury-related pain. Most patients had pain in more than one body region, and the mean [average] severity of pain in the last month was 5.5 on a 10-point scale,&rdquo; the authors write. The occurrence of pain one year after injury was most common in those age 35 to 44 and least common in those 75 to 84. &ldquo;The most common painful areas were joints and extremities (44.3 percent), back (26.2 percent), head (11.5 percent), neck (6.9 percent), abdomen (4.4 percent), chest (3.8 percent) and face (2.8 percent).&rdquo; </p><p> Most (59.3 percent) of those with injury-related pain had three or more painful areas one year after injury, while only 37.3 percent had a single painful area. Patients age 75 to 84 had the fewest number of injury-related painful areas, while those 35 to 44 had the most.</p><p><br />]]></content:encoded>

                <content:encoded><![CDATA[<strong><span class="textbold">Stem cell harvesting from clones could save lives.</span></strong><br />  <p><span class="textjustify">U.S. scientists said Tuesday they have begun work on cloning human embryos to create stem cells, a goal that some find ethically objectionable, according to the <em>Associated Press</em>. </span></p>  <p><span class="textjustify">Dr. George Daley, a leading expert in blood diseases and an executive committee member of the Harvard Stem Cell Institute, is overseeing the work at Children's Hospital Boston, the main pediatric teaching hospital of Harvard Medical School. Daley said he had begun experiments but declined to describe the results, <em>AP</em> reported. </span></p>  <p><span class="textjustify">The Children's Hospital team's goal is to create stem cells for treating blood diseases like sickle-cell anemia, leukemia and other blood disorders. Two other researchers, Douglas Melton and Kevin Eggan, have received approval from a series of review boards to begin similar work, the institute said. Melton plans to focus on diabetes. Eggan plans to work on neurodegenerative conditions like Lou Gehrig's disease. </span></p>  <p><span class="textjustify">By cloning embryos, scientists hope to produce transplant material to treat a variety of diseases. Stem cells can give rise to more specialized cells and tissues that can be genetically matched to patients, avoiding the problem of rejection. </span></p><p>  <span class="textjustify">The University of California, San Francisco, is also pursuing the cloning of human embryos, according to <em>AP</em>, joining the race among a small group of researchers in this controversial pursuit.</span></p><p>&nbsp;</p><span class="textjustify"></span>]]></content:encoded>
